The Arizona Medical Power of Attorney Form is a variation on the Power of Attorney Forms used specifically to allow the Attorney-in-Fact to make decisions for the Principal in matters of the Principal’s medical care and health. It can also allow the Attorney-in-Fact to make decisions regarding the Principal’s funeral and other similar decisions, in the event of the Principals’ death. In Arizona, these are known as Health Care Power of Attorney.
A Living Will, which lays out what end of life decisions the Principal would like made on their behalf, can be signed as part of a Medical Power of Attorney Form or it can be a separate form altogether. In either case, a Medical Power of Attorney Form does not give the Attorney-in-Fact power over any non-medical decisions of the Principal. In Arizona, if there is only one witness to the signing of the Medical Power of Attorney Form, the Attorney-in-Fact cannot be related to the Principal, either through blood or adoption.
