The Connecticut Durable Power of Attorney Form is an agreement that not only gives the Attorney-in-Fact the ability to make decisions on behalf of the Principal, but also continues working after the Principal is incapacitated. This kind of agreement can sometimes be set to only work once the Principal has been incapacitated and can no longer make decisions for themselves, but that is not always the case.

Note that, regardless of the terms of this agreement, this kind of form cannot be used to give the Attorney-in-Fact the ability to make medical, health care or End of Life decisions for the Principal. Medical Power of Attorney agreements are governed separately and thus would require a separate form.
