The Hawaii Medical Power of Attorney Form is a unique kind of Limited Power of Attorney Agreement that gives the Attorney-in-Fact the power to make decisions on behalf of the Principal in regards to their health care and medical treatment. These agreements are, more often than not, Springing and will therefore only come into effect when the Principle has become incapacitated.

Note that, under Hawaii State Law, all Medical Power of Attorney Agreements (technically known as Advance Health Care Directives) will include a Living Will. If the Principal does not wish the Attorney-in-Fact to have power over their End of Life care, then they should most likely appoint another Attorney-in-Fact or specifically limit those powers in their Advance Health Care Directive.
