An Indiana Medical Power of Attorney Form is designed to allow the Attorney-in-Fact to make decisions for the Principal in matters of health care and medical treatments, and is the only kind of form that can grant that power. These kinds of agreements are typically Springing, and will therefore only come into effect once the Principal has been declared incapable of making their own decisions by a doctor.

Note that, under Indiana State Law, a Medical Power of Attorney (known legally in Indiana under the rather cumbersome name of a Durable Power of Attorney for Health Care and Appointment of Health Care Representative) is a separate agreement from a Living Will Declaration. If the Principal wishes their Attorney-in-Fact to have power over their End of Life care and arrangements, they must draft and sign a separate agreement.
