A Missouri Medical Power of Attorney Form is used to give the Attorney-in-Fact the power to make decisions about the Principal’s health care or medical treatments. These agreements are unique, as Medical Power of Attorney agreements are governed differently from all other types of Power of Attorney agreement.

Under Missouri state law, a Medical Power of Attorney form (which is legally known as a Durable Power of Attorney for Health Care) does not include a Living Will, which can also be known as a Declaration. Many Principals will want to make their wishes regarding End of Life care known at the same time they sign their Medical Power of Attorney, but this is not required and a Principal could even name a different person to manage their End of Life care.
