Ohio Medical Power of Attorney Form

The Ohio Medical Power of Attorney Form is designed to give the Agent the legal power to make decisions about the Principal’s medical treatment and care. While it is technically a Durable Limited Power of Attorney, this power cannot be folded into another Power of Attorney agreement, as Medical Power of Attorney is governed separately from all other Power of Attorney.

In Ohio, a Medical Power of Attorney Form (which is also known, under Ohio state law, as a Durable Power of Attorney for Health Care) is a separate agreement from a Living Will Declaration. Therefore, if the Principal wishes to give their Agent the power to manage their End of Life care, they will need to draft and sign a separate agreement.
