The California Medical Power of Attorney Form is a Power of Attorney agreement used specifically, and only, for granting the Attorney-in-Fact power over the Principal’s medical and health care decisions. This is typically put in place when the Principal is unable to make decisions for themselves, and can often cover End of Life or Funeral arrangements.
Note that, under California state law, a Medical Power of Attorney Agreement is called an Advance Health Care Directive and is combined with a Living Will. As such it will also need to cover the Principal’s desires and instructions for what should be done with them should they be rendered comatose and similar End of Life options. In addition, a Medial Power of Attorney agreement, like a Durable Power of Attorney Agreement is required to be witnessed by two (2) people in California.
