The Connecticut Medical Power of Attorney Form is the only Power of Attorney form that can give the Attorney-in-Fact the power to make decisions about the Principal’s medical procedures and health care. This is primarily used when the Principal is incapacitated (IE, declared comatose or diagnosed with dementia) and is unable to make medical decisions for themselves.

Note that End of Life care is, in Connecticut, sometimes covered in a separate form, although not always. Under Connecticut state law a Medical Power of Attorney form (legally known as an Appointment of Health Care Representative) can either be separate from a Living Will (legally called a Document Concerning Health Care and With­holding or Withdrawal of Life Support Systems) or it can be combined into a single agreement known as Health Care Instructions and Appointment of Health Care Representative.
