The Nebraska Medical Power of Attorney Form is used to give the Attorney-in-Fact the power to make decisions about the Principal’s health care and medical treatment. This is the only kind of agreement that can give the Attorney-in-Fact this power, as Medical Powers of Attorney agreements are governed differently from other Power of Attorney agreements.

In Nebraska, a Durable Power of Attorney for Health Care is a completely separate agreement from a Living Will Declaration. If the Principal wants their Attorney-in-Fact to have the authority to manage their End of Life care and make funeral arrangements, they will need to draft a Living Will in addition to the Medical Power of Attorney Agreement.
