A Puerto Rico Medical Power of Attorney Form is used when the Principal needs an Attorney-in-Fact who can manage and make decisions about their health care and medical treatments. This is the only agreement that can give the Attorney-in-Fact these powers, as Medical Power of Attorneys are governed differently from all other Powers of Attorney; These powers cannot be folded into another Power of Attorney form.

In Puerto Rico, a Medical Power of Attorney form does not include a Living Will Declaration. If the Principal wants their Attorney-in-Fact to have power over their End of Life care and similar arrangements, they may want to draft a Living Will at the same time.
